Combating Alert Fatigue to Improve Clinician Adoption

چکیده

confronting healthcare organizations in their quest to encourage enthusiastic adoption of CPOE and clinical decision support (CDS) at the point of care. If too many alerts are triggered when medications or tests are being order, the likelihood is very high physicians will eventually tune out or actively override even high severity alerts. Alert fatigue can also cause physicians to bypass or remain skeptical of CPOE, resulting in low adoption rates that impact outcomes and the hospital’s return on its technology investment. This is typically the end result when, as one hospital CMIO noted, alert fatigue drives clinicians to view CPOE as a “challenge to their autonomy or the flavor of the month.”

Effects of workload, work complexity, and repeated alerts on alert fatigue in a clinical decision support system

BACKGROUND Although alert fatigue is blamed for high override rates in contemporary clinical decision support systems, the concept of alert fatigue is poorly defined. We tested hypotheses arising from two possible alert fatigue mechanisms: (A) cognitive overload associated with amount of work, complexity of work, and effort distinguishing informative from uninformative alerts, and (B) desensiti...

Drug interaction alert override rates in the Meaningful Use era

Background: Interruptive drug interaction alerts may reduce adverse drug events and are required for Stage I Meaningful Use attestation. For the last decade override rates have been very high. Despite their widespread use in commercial EHR systems, previously described interventions to improve alert frequency and acceptance have not been well studied. Objectives: (1) To measure override rates o...

Application of Information Technology: Improving Acceptance of Computerized Prescribing Alerts in Ambulatory Care

Computerized drug prescribing alerts can improve patient safety, but are often overridden because of poor specificity and alert overload. Our objective was to improve clinician acceptance of drug alerts by designing a selective set of drug alerts for the ambulatory care setting and minimizing workflow disruptions by designating only critical to high-severity alerts to be interruptive to clinici...

Optical Flow and Driver's Kinematics Analysis for State of Alert Sensing

Road accident statistics from different countries show that a significant number of accidents occur due to driver's fatigue and lack of awareness to traffic conditions. In particular, about 60% of the accidents in which long haul truck and bus drivers are involved are attributed to drowsiness and fatigue. It is thus fundamental to improve non-invasive systems for sensing a driver's state of ale...
